“Buffy the Vampire Slayer” actor Nicholas Brendon died on March 20 at age 54 due to natural causes stemming from atherosclerotic and hypertensive cardiovascular disease, according to Putnam County coroner’s office findings.
Coroner Todd Zeiner reported that the actor suffered from a “mechanism of death” involving 90% blockage of the right coronary artery, with acute pneumonia and a previous myocardial infarction identified as significant contributing factors in the official report. Autopsy results revealed a “markedly enlarged heart” and severe stenosis of the right coronary artery. Medical examiners also documented moderate stenosis of both the left anterior descending and left circumflex arteries, alongside inflammation of the small bowel. Zeiner noted that this small bowel inflammation was “most likely secondary to ischemic changes related to cardiogenic shock.”
The American Heart Association explains that atherosclerotic cardiovascular disease—characterized by plaque buildup within artery walls—remains a leading global cause of death.
In the months preceding his death, Brendon’s health condition visibly deteriorated. Zeiner referenced videos filmed from the actor’s home deck, noting he was “markedly coughing” and had a “raspy voice,” which provided evidence for his declining health. A longtime friend, Theresa Fortier, called 911 after finding Brendon at his residence. She reported he had been self-medicating with over-the-counter medications for a persistent cough and had complained of chest pain. Zeiner wrote that Brendon, a longtime smoker, declined hospitalization due to recent back surgery and apprehension about medical procedures.
Fortier also stated that while Brendon had experienced a heart attack years earlier, he had refused further treatment at that time. Law enforcement confirmed “no obvious signs of foul play” during the investigation, with the coroner cataloging the residence as having “nothing out of place or disturbed.”
Brendon’s health struggles spanned years, including a cardiac incident in August 2022 requiring an emergency room visit for tachycardia and arrhythmia. In 2021, his representative noted he was experiencing “deep-seated medical problems and immense pain” around the time of a legal charge involving prescription fraud.
Brendon is best known for his role as Xander Harris on “Buffy the Vampire Slayer,” appearing alongside Sarah Michelle Gellar and Alyson Hannigan from 1997 to 2003. His final months were marked by legal disputes, including a December 2025 lawsuit seeking over $120,000 regarding home renovations and a court hearing just eight days before his death where he was accused of mistreating contractors.
Following his passing, Brendon’s family released an Instagram statement: “We are heartbroken to share the passing of our brother and son, Nicholas Brendon. He passed in his sleep of natural causes… Those who truly knew him understood that his art was one of the purest reflections of who he was. While it’s no secret that Nicholas had struggles in the past, he was on medications and treatment to manage his diagnosis and he was optimistic about the future at the time of his passing.” The family also requested privacy during their grief.
